Participant or Parent of Participant agrees as follows:

I understand that if my purchase is a package of services, that special pricing was made available to me to purchase these services in a bundle. Therefore I acknowledge that partial refunds will not be available for unused portions of the package.

I understand that the agreement to deliver the defined training services will expire 18 months after the date of the enrollment in Colorado. I understand that the agreement to deliver the defined training services will expire 12 months after the date of the enrollment in California.

I agree to provide evidence that my vehicle is fully insured during the period of my participation in the MasterDrive training program, and my vehicle will not be equipped with studded tires.

I accept and assume full responsibility for my actions during all MasterDrive training. The participant's attendance is voluntary, and he or she elects to participate in spite of the inherent risks.

I do consent for the use of photographs and/or videotapes taken of me, for the purposes of promotion, marketing, or distribution in any manner by MasterDrive, unless I have provided written documentation stating I prefer no pictures or videos be used.

By signing this document, I acknowledge that if I am injured or if my property is damaged during my participation in this training, I may be found by a court of law to have waived my right to maintain a lawsuit against MasterDrive, its agents, instructors, insurers, sponsors, employees, successors, and assigns.

I acknowledge that if I may have provided my personal and credit card account information to MasterDrive, solely for the purpose of allowing fees to be charged on my credit card for services provided. If I have entered into a payment plan agreement, I give MasterDrive permission to retain my personal and credit card information for ongoing charges pursuant to the payment plan that I have entered into with MasterDrive.

I agree to endeavor to keep scheduled training events and understand that there may be rescheduling fees unless MasterDrive is notified more than 48 hours in advance of a training event. Fees are as follows: $50 for the classroom, on-street, or Street Prep sessions, and $150 for Driving Camp. Fees may be waived in cases of emergency or health-related situations.

By signing this document, I agree to receive emails from MasterDrive, including a reminder of training events, inventory releases, and communications concerning changes in curriculum.
